Note for parents of pupils going on the trip to France. The form asks for a valid Form E111; this has been replaced with the EHIC (European Health Insurance Card) and is valid for 5 years, rather than 1 year as per the E111. Parents might be interested to know that you can apply online via www.direct.gov.uk and follow the links for Travel. You only need to supply the students National Insurance Number, or National Health Number for ID, and existing EHIC cards can also be renewed online (these were initially valid for 3 years). The process is very simple, but it saves a trip to the Post Office.
